You can access all of your visits with 98point6 in the app 24/7 by signing in and navigating to the history section. From there you can see your visits organized by date, and view your Care Plans and transcripts.
If you would like a digital copy of your records, please complete the Authorization Form. Please send the completed and signed form and a photocopy of your government issued ID back to us as an attachment through our Contact Us page. You will need to include the phone number that your account is attached to as your contact information on the form as well. (Please note, you will need to create a secure Zendesk Support account and login in order to receive and reply to messages from our Support Teams.
